Reporting to the Assistant Director, Living in Place, the Clinical Coordinator, Living in Place, provides clinical support to Queens based, Aging Services programs at Breaking Ground using a Critical Time Intervention (CTI)-informed model. This role provides time-limited services focused on housing retention, resource development, and community integration intended to support older adults with aging safely in the community. Services include information and referral, benefits application and navigation assistance, resource development support, and community integration programming. The case management for Aging Services is typically a 3–6 month engagement and on a referral basis. The Clinical Coordinator maintains a caseload of 30-40 clients and serves as a member of the Tenant 360 management team. This position has a 35-hour work week and is fully in-person, dividing time across the Breaking Ground building portfolio. The position may require occasional evening or weekend hours to support tenant events.
