Requirements

• Must possess or be eligible to obtain licensure as a Social Worker or Social Work Associate through the South Dakota Board of Social Work Examiners, with in 6 months of employment.
• Obtaining and maintaining licensure is a requirement of this position.
• A bachelor’s degree in social work or a related field, is required.
• Must be at least 21 years of age, is required.
• Willingness to submit to record checks to detect criminal convictions of behavior harmful towards children, spousal abuse, distributing drugs or sex crimes.
• Requires a valid auto-insurance, a valid driver’s license or the ability to obtain one with approved driver status.

Nice To Haves

• Licensed Social Worker or Licensed Social Work Associate through the South Dakota Board of Social Work Examiners is preferred.
• Experience with client Medication Administration Records (MARs), is preferred.

Responsibilities

• Helps develop individualized case plans
• Conducts ongoing assessments
• Connects youth with appropriate community resources
• Identifying the needs of youth and their families
• Supporting their growth
• Fostering positive change in their lives
• Maintaining accurate case documentation
• Building collaborative relationships with community partners and service providers
• Coordinating medical, dental, and vision appointments
• Ensuring annual exams are completed
• Supporting medication management processes in collaboration with nursing staff and pharmacy providers
• Obtaining medical records and treatment consents
• Reviewing Medication Administration Records (MARs) for accuracy
• Transporting clients to appointments as needed
