Requirements

• Must possess or be eligible to obtain licensure as a Social Worker or Social Work Associate through the South Dakota Board of Social Work Examiners, within 6 months of employment.
• Obtaining and maintaining licensure is a requirement of this position.
• A bachelor’s degree in social work or a related field, is required.
• Must be at least 21 years of age, is required.
• Willingness to submit to record checks to detect criminal convictions of behavior harmful towards children, spousal abuse, distributing drugs or sex crimes.
• Requires a valid auto-insurance, a valid driver’s license or the ability to obtain one with approved driver status.

Nice To Haves

• Licensed Social Worker or Licensed Social Work Associate through the South Dakota Board of Social Work Examiners is preferred.
• Experience with client Medication Administration Records (MARs), is preferred.

Responsibilities

• Develop individualized case plans
• Conduct ongoing assessments
• Connect youth with appropriate community resources
• Identify the needs of youth and their families
• Support their growth
• Foster positive change in their lives
• Maintain accurate case documentation
• Build collaborative relationships with community partners and service providers
• Coordinate medical, dental, and vision appointments
• Ensure annual exams are completed
• Support medication management processes in collaboration with nursing staff and pharmacy providers
• Obtain medical records and treatment consents
• Review Medication Administration Records (MARs) for accuracy
• Transport clients to appointments as needed
