About The Position

Felton Institute is seeking a Clinical Case Manager for its Older Adult Full Service Program within the Senior Division. This program offers a full continuum of mental health services for older adults, including outpatient clinics, community outreach, and supportive housing. The Full-Service Wellness Partnership specifically focuses on client-centered, recovery-oriented treatment for individuals with severe psychiatric disorders, addressing co-occurring medical and substance abuse issues, homelessness, and social isolation. The multidisciplinary team, including a psychiatrist, psychiatric nurse practitioner, clinical case manager, and peer/community specialist, works collaboratively to provide extensive outreach, therapy, and case management to reduce hospitalizations and support community living. The program is committed to evidence-based practices and behavioral health research.

Requirements

  • Master's degree or Licensed in Social Work; Marriage and Family Therapy, or Professional Counseling and registered with active license with CA BBS, including ASWs, AMFTs, and APCCs.
  • A minimum of 2 years of case management and mental health treatment for at-risk older adults, including co-occurring disorders (master’s level internship experience considered)
  • Moderate to comprehensive understanding of Specialty Mental Health Services under the Department of Health Care services definitions, in addition to skills in assessment, treatment planning, and mental health services pertaining to individuals living with serious mental illness
  • Strong recovery model approach to care and ability to form strong therapeutic relationships with clients from diverse backgrounds and who often are underserved by traditional care models
  • Ability to effectively diagnose and make clinical rationales for psychiatric care utilizing the DSM 5 and other measures including social determinants of health and functional impairment screenings where appropriate
  • Actively practices cultural humility and promotes a warm and welcoming environment for culturally and gender diverse individuals
  • Excellent communication skills both in written and verbal formats
  • Ability to travel within SF to meet clients in a variety of community settings including SROs, hospitals, shelters, and other settings required
  • Ability to be a team player with a welcoming attitude.

Responsibilities

  • Work closely with interdisciplinary team.
  • Provide crisis intervention to prevent hospitalization, and if necessary, facilitates involuntary hospitalization.
  • Perform intakes, assessments, and service plan development for older adults with psychiatric and dual diagnosis disabilities in a Felton clinic, community, and/or in their homes.
  • Provide clinical case management, which includes individual or group therapy, case management, connection to primary care, and coordination with other providers.
  • Maintains clinical records of each client contact or service in accordance with mandated standards of Medi-Cal, Medicare, and County Behavioral Health Services, and meets requirements of timely access for client care, documentation, and billing requirements.
  • Participates in team meetings and case conferences through presentation of cases, discussion of clinical issues and exchange of information and ideas.
  • Commits to ongoing training, including developing specializations in evidence-based treatments and practices and the geriatric specialization of mental health services, as provided by the Senior Division and Felton Institute.
  • On-call 24-hour crisis phone line duty on a weekly rotated shift amongst team of six people; extra pay awarded for shift.
  • Other duties and responsibilities as assigned.
