About The Position

The Clinical Case Manager provides integrated case management and clinical support to survivors residing in shelter. This role combines traditional case management with therapeutic intervention, including crisis stabilization, brief individual support, and facilitation of structured, trauma-informed therapeutic groups focused on emotional regulation, stabilization, and symptom management.

Requirements

  • Master’s degree in Social Work, Counseling, or related field
  • Licensure required (LSW, LPC, LISW, LPCC)
  • Experience with trauma, crisis intervention, and/or group facilitation
  • Maintain valid Ohio driver’s license and ability to meet agency requirements for driving insurability.
  • Must be able to meet agency requirements for criminal background check (Ohio Bureau of Criminal Investigations and Federal Bureau of Investigations).

Responsibilities

  • Provide trauma-informed case management, including safety planning, housing stabilization, and service coordination
  • Conduct clinical assessments to identify mental health needs, risk factors, and service priorities
  • Deliver therapeutic crisis intervention and short-term clinical support to residents experiencing acute distress
  • Facilitate structured, trauma-informed therapeutic groups focused on emotional stabilization and skill development to support recovery from trauma and co-occurring conditions.
  • Collaborate with therapists to triage higher-acuity clients and ensure appropriate referrals for ongoing care
  • Maintain accurate, timely clinical documentation in accordance with agency, funder, and confidentiality requirements
  • Partner with shelter staff to support consistent, trauma-informed responses to resident needs
  • Support discharge and transition planning to ensure continuity of care following shelter exit
  • Support shelter operations, including overall safety, conflict resolution, de-escalation, and client engagement.
  • Adheres to Service Excellence standards at all times
  • Represent Hope & Healing Survivor Resource Center by participating in community activities, coalitions and taskforce groups as requested or assigned by supervisor.
  • Complies with agency policies and procedures, federal and state requirements, CARF standards, documentation requirements, and educational/certification/registry requirements.
  • Performs other duties as assigned.
