Under supervision of the Program Manager, the Clinical Case Manager participates in a multidisciplinary team, including master's level clinicians, registered nurses, peer specialists, interns, and psychiatric medication providers, serving as the lead point of responsibility for enacting client care. The CHSP Caseload is capped at a lower level to allow for intensive services to stabilize individuals who are experience acute symptoms and related risk factors. The CHSP clinician evaluates needs of clients, and develops comprehensive clinical assessments, treatment plans, and coordinates care among the team. Services may include individual and group therapy, crisis intervention, case management, client advocacy, and engagement strategies. The CHSP clinician also maintains accurate detailed records of client contacts through Cal-AIM (Medi-Cal) and Medicare standards as well as county and state regulations. Additional duties as needed.
