About The Position

The Clinical Care Specialist plays an essential role in delivering high-quality member care by coordinating and implementing clinical services tailored to individual member needs. The Clinical Care Specialist collaborates closely with multidisciplinary teams to ensure comprehensive care plans are developed and executed effectively, improving member outcomes and satisfaction. The Clinical Care Specialist will serve as a key liaison between members, families, and healthcare providers, facilitating clear communication and education regarding treatment options and care processes. Additionally, The Clinical Care Specialist requires continuous assessment and monitoring of member progress, identifying any changes in condition and adjusting care plans accordingly. Ultimately, the Clinical Care Specialist contributes to the overall efficiency and quality of healthcare delivery by integrating clinical expertise with compassionate member support.

Requirements

  • Bachelor’s degree in Nursing, Healthcare Administration, or a related clinical field.
  • Active, unrestricted Florida RN License or licensure by endorsement is required.
  • 2 years of experience in a clinical healthcare environment.
  • Strong knowledge of clinical care standards, member safety protocols, and healthcare regulations.
  • Proficiency in electronic health record (EHR) systems, clinical decision support tools, and basic computer applications.
  • Relevant experience may substitute for the educational requirement on a year-for-year basis.

Nice To Haves

  • Master’s degree in Nursing, Public Health, or Healthcare Management.
  • Experience working in specialized clinical settings such as inpatient, outpatient clinics, rehabilitation.
  • Familiarity with quality improvement methodologies and member-centered care models.
  • Bilingual (English/Spanish) abilities and experience working with diverse member populations.

Responsibilities

  • Conduct thorough member assessments to determine clinical needs and review proposed care plan and if needed, develop individualized care plans.
  • Coordinate with physicians, nurses, and other healthcare professionals to implement and monitor treatment protocols.
  • Provide member education and counseling to promote understanding of health conditions and adherence to care plans.
  • Document member interactions, progress, and any changes in condition accurately in medical records.
  • Participate in quality improvement initiatives and ensure compliance with healthcare regulations and standards.
  • Respond promptly to member concerns and emergencies, facilitating timely interventions.
  • Support discharge planning and follow-up care to ensure continuity and prevent readmissions.
