About The Position

Fayette County Behavioral Health Administration has an immediate need for a Clinical Care Reviewer (County Care Management Manager). This position provides telephone assessment and referral services based on required guidelines and HealthChoices standards to help people in our county find the best programs and services to meet their individual and family needs. We support consumers who are in need of psychiatric and behavioral services provided by a mental health professional, or in need of drug and alcohol treatment services provided by a drug and alcohol addictions professional. Requirements

  • Two years of professional mental health clinical experience and possession of an active license to practice as a Registered Nurse issued by the Pennsylvania State Board of Nursing; or A master's degree in social work, psychology, nursing, or counseling and two years of professional mental health clinical experience.
  • Active license issued by the Pennsylvania Bureau of Professional and Occupational Affairs in one of the following areas: Social Work 20001-Social Worker 20010-Clinical Social Worker 20020-Marrage and Family Therapist 20030-Professional Counselor Psychology 26001-Psychologist Nursing 18001-Registered Nurse 18040-Certified Registered Nurse Practitioner 18170-Clinical Nurse Specialist
  • Meet the PA residency requirement.
  • Must be able to perform essential job functions.
  • Satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.

Responsibilities

  • Conduct Telephonic Assessments: Gather demographic and clinical information necessary to make emergency, urgent, and routine referrals for mental health and substance abuse services
  • Share Information: Provide consumers with information regarding benefits, treatment options, the managed care system, and community resources
  • Determine Care Needs: Review and evaluate the medical appropriateness of psychiatric and behavioral health and substance abuse cases and, in consultation with supervisor, render a certification decision
  • Serve as an Advocate: Support and promote the interests of consumers and families; facilitate coordination of care with other care managers; and conduct follow-up with consumers and service providers
  • Ensure Quality and Credentials: Participate in customer service assessment activities to assure quality; participate in clinical rounds, seminars, and training sessions to maintain licensure and clinical credentials
  • Complete Documentation: Maintain accurate data, referrals, and case documentation in the automated system and client records, in accordance with program standards; prepare required reports as mandated by federal, state or county policies and procedures
