Crawford County Human Services is looking for a Blended Case Manager to join their team. The primary responsibility of this role is to serve children and adults who have been diagnosed with serious and persistent mental illness and who are in need of assistance from other community services. This position provides advanced professional social services work to reduce avoidable inpatient hospitalizations and enhance the quality of community living and independent social functioning for children and adults. Requirements

  • Two years of experience as a County Caseworker 2
  • A bachelor's degree in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and three years of professional social casework experience in a public or private social services agency
  • Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences
  • PA residency requirement is currently waived for this title
  • You must be able to perform essential job functions
  • You must pass a background investigation
  • This position falls under the provisions of the Child Protective Services Law
  • A conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance

Responsibilities

  • Interview consumers, family members, and other involved individuals to complete initial mental health assessments; revaluate and determine need for continued services every six months
  • Develop and implement goal directed service plans based on interview and assessment results; coordinate with mental health program specialists when appropriate
  • Review and evaluate progress notes, treatment and service plans, and other written documentation of services
  • Help consumers gain access to resources such as medical, housing, social, educational, vocational, and other services; provide or coordinate arrangements for transportation
  • Complete forms, case notes, reports, service authorizations and referrals, and correspondence to prepare for medical assistance billing
  • Lead and participate in meetings; adhere to the required principles, regulations, and code of ethics; mandated reporter at all times
