About The Position

Wayne County Behavioral Health & Intellectual Disabilities in Honesdale, PA is hiring a Caseworker 1 – Blended Case Management. This role focuses on guiding individuals through important service systems and helping them receive the supports they need. Requirements

  • Two years of experience as a County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences
  • A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences
  • Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions
  • Must be able to perform essential job functions
  • Pass a background investigation
  • Pass a drug screening
  • Satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance

  • Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.

Responsibilities

  • Create individualized plans that outline supports and services
  • Connect individuals and families with appropriate providers
  • Maintain organized files, documentation, and timely records
  • Assist individuals in obtaining approved services and resources
  • Travel and complete in-person visits in a variety of community settings
  • Stand, walk distances, and climb stairs in buildings where no elevators are present
