About The Position

The Clinical Care Partner is responsible for coordinating patient care appointments, following established standards and practices of population health management. This role involves calling patients for follow-up appointments after hospital admissions and contacting patients identified by the Quality Department as high risk or needing appointments to address Quality measures. The position relies on established guidelines and prior experience to accomplish tasks, working under the close supervision of the Clinic Manager and Quality Manager.
