About The Position

This role provides field-based clinical leadership and support to community teams, working in close partnership with General Managers, Directors of Wellness (DOW), and operational leaders to ensure care delivery meets organizational standards, regulatory requirements, and evolving resident needs. This role is a regional support role - it requires travel to and working out of our Retirement Sites in Ontario. Specifically but not limited to Ottawa, Perth, Kingston, Markham and Belleville. The successful candidate is required to have access to a reliable vehicle and a valid G license (travel reimbursement applies).

Requirements

  • Registered Nurse (RN), Registered Practical Nurse (RPN), or Licensed Practical Nurse (LPN) in good standing with the applicable regulatory body
  • Minimum 3–5 years’ experience in seniors’ care, retirement living, long-term care, or a related healthcare environment
  • Leadership experience in a clinical or wellness role (e.g., Director of Wellness, Director of Care, Associate Director of Care)
  • Strong knowledge of gerontology, dementia care, and resident-centered care practices
  • Knowledge of applicable regulatory requirements across Retirement settings
  • Experience with clinical assessments, care planning, and quality improvement processes
  • Strong clinical judgment and problem-solving skills
  • Ability to coach, mentor, and influence leaders and teams
  • Commitment to quality, resident experience, and continuous improvement
  • Access to a reliable vehicle and a valid G license

Responsibilities

  • In collaboration with the Vice President, Regional Operations, General Managers, and Directors of Wellness, provide ongoing clinical support to communities to ensure care meets and exceeds organizational and regulatory expectations.
  • Support the assessment of resident care needs and the development and implementation of individualized service plans.
  • Monitor resident outcomes and incidents, supporting teams to implement appropriate interventions and prevent recurrence.
  • Support communities in identifying and responding to changes in resident health status and care needs.
  • Support the implementation and ongoing execution of the Retirement care and wellness program across communities.
  • Partner with community teams to ensure care delivery aligns with service plans, staffing models, and care fee structures.
  • Conduct regular site visits, chart audits, and quality reviews to assess compliance with organizational standards and regulatory requirements.
  • Support communities in the development and execution of quality improvement and compliance action plans.
  • Identify clinical risks and partner with community teams to implement mitigation strategies.
  • Support the implementation of IPAC practices and protocols within communities.
  • Facilitate education sessions related to clinical care, quality, IPAC, and regulatory requirements.
