About The Position

The Drexel University Counseling Center is seeking a year-round, full-time Clinical Care Manager. This role focuses on supporting students who require longer-term or more specialized treatment, providing coaching and skills-based training for students navigating care, managing referrals and follow-ups with campus and community partners, and collaborating with community mental health providers. The Clinical Care Manager will ensure clients' off-campus needs are met and provide consistent follow-up.

Requirements

  • Minimum of a Master's Degree in Social Work (LSW), Psychology or related field or the equivalent combination of education and work experience.
  • Minimum of 3 years of experience in mental health counseling, case management, care coordination, preferably in a higher education setting.
  • Experience working with multicultural and marginalized identities.
  • Strong interpersonal skills.
  • Excellent written and verbal communication and time management skills.
  • Ability to work independently and establish relationships, and function within a team.
  • Ability to effectively manage students with significant mental health needs.

Responsibilities

  • Provides case management for students requesting or recommended for a transfer of care out of DUCC services.
  • Serves as a bridge to appropriate community resources, facilitating access and communication, and monitoring service delivery.
  • Assists with coordination of hospitalization and discharge planning and advocates for students in accessing care.
  • Assists in tracking students referred to the community and communicates with them as needed about continuity of care.
  • Consults with parents, faculty, and staff when appropriate to ensure quality care to students.
  • Develops and maintains comprehensive knowledge of community providers and agencies to expand and maintain mental health referral networks and relationships.
  • Assists in improving internal and external referral processes.
  • Compiles, maintains, and updates referral resource information, including an updated database.
  • Provides follow-up contact for clients of Counseling & Health engaged in treatment with community medical providers.
  • Maintains timely records regarding client status, in keeping with Counseling Center record-keeping policies.
  • Provides ‘bridge’ services as needed.
  • Other duties as assigned.
