About The Position

The primary purpose of the Clinical Care Coordinator position is to serve as the coordinator and practitioner of the clinic’s healthcare team(s) within the Women’s and Children’s Service Lines, utilizing specialized knowledge, skills, and competencies in behavioral sciences, education counseling, and/or patient management. Develops patient programs and course curricula based on needs assessments; plans and coordinates the delivery of comprehensive services to patients in collaboration with all professional components of the clinic and in accordance with accepted standards of care and department protocols.

Requirements

  • Bachelor’s degree in nursing (BSN) or a related health care field.
  • Must hold CPR certification.
  • Be knowledgeable in Managed Care and Healthcare Industry trends.

Nice To Haves

  • A master’s degree in nursing, Health Care Administration, Public Health, or a related field is preferred.
  • A minimum of three (3) years of full-time experience in a hospital-based outpatient practice is preferred.
  • Effective team leadership and interpersonal skills are required.
  • National Certification in Obstetrics or Pediatric Nursing preferred.
  • Two (2) years’ experience in Maternal-Child;OB/GYN specialties preferred to include but not limited to, Pediatrics, Reproductive Choice.
  • Additional clinical experience can substitute for the degree.

Responsibilities

  • Provides educational services and programs for patients, families, and staff within the framework of the department.
  • Meets the needs of select patients through a care management approach.
  • Serves as an active member of a consortium of providers of care within the clinic to standardize the management of the patients.
  • Participates in the development of staff associated with the management of the disease.
  • Maintains a high standard of professional responsibility and performance.
  • Participates in research and performance improvement activities
