Requirements

  • Current Washington State Certified Nursing Assistant (CNA) or Home Care Aide (HCA) certification.
  • Current CPR/BLS certification.
  • Minimum two years of caregiving experience.
  • Strong leadership, organizational, communication, and problem-solving skills.
  • Ability to independently prioritize multiple responsibilities.
  • Valid Washington State driver's license, reliable transportation, and willingness to travel throughout the assigned service area.

Nice To Haves

  • Home Care leadership or supervisory experience.
  • Nurse Delegation certification.
  • Experience completing client assessments and supervisory visits.
  • Experience training and mentoring caregivers.
  • Experience supporting medically complex clients, dementia, Parkinson's disease, hospice, or developmental disabilities.

Responsibilities

  • Manage an assigned caseload of home care clients.
  • Complete Start of Care assessments, supervisory visits, and recurring client assessments.
  • Develop, implement, and update individualized Plans of Care.
  • Build strong relationships with clients, families, providers, and referral partners.
  • Monitor client progress and identify changing care needs.
  • Build proactive backup staffing plans for assigned clients.
  • Ensure required documentation is maintained in client homes.
  • Provide direct client care and field support when operationally needed.
  • Orient, train, mentor, and support caregivers.
  • Complete caregiver supervisory visits and competency evaluations.
  • Ensure caregivers understand and follow each client's individualized Plan of Care.
  • Promote accountability, teamwork, and high-quality care.
  • Provide coaching, education, and ongoing performance support.
  • Maintain compliance with agency policies, payer requirements, and Washington State regulations.
  • Coordinate physician orders and care plan updates.
  • Monitor required supervisory visits, reassessments, and care periods.
  • Complete accurate, timely, and compliant documentation.
  • Participate in quality assurance and performance improvement initiatives.
  • Collaborate with scheduling to maintain appropriate staffing.
  • Anticipate staffing challenges and develop proactive coverage plans.
  • Assist with opening new cases and coordinating client transitions.
  • Participate in interdisciplinary meetings and weekly case conferences.
  • Serve as the primary liaison between clients, families, caregivers, providers, referral partners, and agency leadership.
  • Communicate changes in client condition promptly.
  • Promote continuity of care through effective communication and teamwork.
