About The Position

This is a Part Time position at the Chubbuck Clinic in Chubbuck, ID. The role involves telephone triage for patient concerns, performing patient follow-up and coordinating care, refilling medications, tracking patient recalls and outpatient referrals, and acting as a float to assist with in-person patient care during staff shortages. The position also requires participation in meetings, continuous quality improvement activities, and assisting with outreach events. Responsibilities include documenting client encounters, completing monthly reports, ensuring accurate data collection, coaching patients on chronic health condition management, assisting patients in understanding care plans, motivating patients towards better health, and helping patients access health-related services. The role requires compliance with Health West Inc.'s policies and procedures.

Requirements

  • High School Diploma or Equivalent
  • Graduation from a Medical Assistant Program or Equivalent
  • Graduation from a Practical Nursing Program or Equivalent
  • One year recent experience in a health care setting preferred.
  • Current National Medical Assistant Certification required.
  • Current Idaho Practical Nursing License or equivalent required.
  • Current certification in BLS.

Responsibilities

  • Telephone triage for patient concerns/questions
  • Perform patient follow-up and coordinates care (e.g. obtain prior authorizations, follow-up on abnormal results, manage appropriate buckets in EMR, etc.), per clinic protocol
  • Refill patient medications per clinic protocol
  • Enter in and tracks patient recalls as appropriate (childhood and adult immunizations, future blood work, abnormal paps, colonoscopies, etc.)
  • Track outpatient referrals, per policy and procedure
  • Review and coordinate with other departments and integrate work with other staff to ensure timely and accurate patient flow
  • Act as float to help fill in during staff shortages for in-person patient care in the clinical setting; follow workflows, policy procedures set for clinical nursing
  • Participate in meetings
  • Participate in continuous quality improvement activities
  • Assist with outreach activities such as health fairs, when needed
  • Document client encounters from outreach events, activities, service plans, and outcomes achieved by client in an effective manner; completes and submits monthly reports; and ensures accurate data collection for quality measures and other reporting purpose
  • Coach patients in effective management of their chronic health conditions and self-care.
  • Assist patient in understanding care plans and instructions.
  • Motivate patients/clients to be active and engage event participants in their health and overall wellbeing.
  • Assist patients in accessing health-related services to appropriate medical and specialty providers and overcoming barriers to obtaining needed medical care and /or social service
