About The Position

Responsible for the coordination and completion of a specified project or projects. Oversees all aspects of projects, defines project scope, goals and deliverables that support business goals. Sets deadlines, assigns responsibilities, and monitors and summarizes progress of project. Manages schedules and ensures all assignment areas are covered each day. As Project Specialist for Inpatient Discharge Services, responsibilities will include identifying patients eligible for discharge services, defining scope of services, assigning work to team members, monitoring outcomes of discharge visits, and identifying areas for improvement. Continuous evaluation of program will be required with recommendations for process changes and expanding scope.

Requirements

  • Bachelor’s degree from an accredited college or university Or equivalent combination of education and experience.

Responsibilities

  • Primarily serve as the Clinical Assistant for the Hospital Medicine Unit (HMU) physicians and APPs providing proactive administrative support to promote HMU patient care progression and discharge.
  • Attend daily multidisciplinary rounds (MDR) to discuss patient progress and identify discharge barriers.
  • Collect medical history.
  • Document and update admission and discharge information in EPIC. Accurately document discharge plans, referrals, and patient/family education, as applicable.
  • Prepare discharge instructions, as directed.
  • Troubleshoot delayed imaging or procedures affecting patient discharge.
  • Communicate plans with the patient and their family ensuring that the patient and the family have an understanding of these.
  • Assist with other aspects of care such as verifying that all arrangements and equipment needed are in place for discharge.
  • Coordinate internal discharge follow up care for a variety of Primary and Specialty care practices.
  • Coordinate external discharge follow up care.
  • Call patients post discharge to check on health condition and confirm appointments. Escalate to clinical leaders as needed.
  • Track all work completed within detailed spreadsheet and provide daily update to supervisor.
  • Communicate with project stakeholders on an ongoing basis.
  • Identify and resolve issues and conflicts within the project team.
  • Proactively manage changes in project scope, identify potential crises, and devise contingency plans.
  • Support internal department initiatives and requirements outside the scope of specific project work.
  • Establishes and implements policies and procedures to ensure that operational scope are addressed and properly communicates outcome measures to team members.
