About The Position

Horizon Health, in partnership with Ashtabula Regional Medical Center, is seeking a Clinical Assessment Coordinator for their inpatient behavioral health unit in Ashtabula, OH. This role focuses on the 17-bed adult/geriatric inpatient psychiatric unit. The Clinical Assessment Coordinator is responsible for managing the referral development and pre-admission patient assessment process, as well as conducting clinical assessments for potential clients. For those with a MA/MSW, this role includes completing specialized clinical evaluations, diagnosing behavioral health conditions, and developing moderate to complex treatment plans.

Requirements

  • Master’s degree required
  • RN, LPC, LCSW or LMSW required
  • 3 – 5 years of related experience
  • Proficient in software applications
  • Ability to read and comprehend moderate to complex instructions and correspondence
  • Ability to effectively present information in one-on-one and small group situations to patients and their families, facility leadership, employees’ clients, providers, vendors, etc.
  • Ability to write detailed correspondence
  • Excellent verbal and written communications skills
  • Ability to Prioritize Multiple Tasks

Responsibilities

  • Diagnose, implement, coordinate, and evaluate clinical admission, referral, and/or discharge processes.
  • Conduct on and off-site patient assessments, including appropriate test administration, interpretation, and recommendations.
  • Respond to intake calls and complete admission paperwork.
  • Participate in the development of treatment plans in coordination with the Treatment team.
  • Coordinate treatment with physicians and other health and social agencies and participate as an active member of the interdisciplinary team.
  • Utilize assessment tools including, but not limited to Geriatric Depression Scale, Zung, LUMS, alcohol screening tool(s) under authorization/supervision of RN, LPC or LMSW.
  • Communicate with referral sources including family members, legal system, psychiatrists, primary care physicians, and other treatment facilities.
  • Review newly admitted patient accounts for eligibility and authorization daily.
  • Maintain schedule for filing court paperwork and hearings.
  • Work closely with court personnel to maintain good relationships.
  • Enter required data into HorizonPlus system and generate monthly reports to track and trend referral, admission, and discharge data.
  • Utilize data to complete program’s key statistical reports and generate reports as needed for the S/VPO, VPCE, program director and client hospital leadership when requested.
  • Respond to requests for general information about the Program’s treatment program and provide information about appropriate topics in response to community requests.
  • Inform Program Director, S/VPO and VPCE of any potential adverse publicity.
  • Perform other duties as assigned/required by this position.
