About The Position

Provides a team approach in coordination with providers and support staff in the provision of care in the clinic setting. Effective communication with provider/nurse teams. The Registered Nurse (RN) will provide a variety of tasks including but not limited to, annual wellness visits, care coordination, assistance with treatments and procedures, education, injections, triage, medication refills, and assisting the Physicians/Advanced Practice Providers with patient care as needed. Maintain a high standard of quality healthcare. Practice within the scope of their licensure. Works in collaboration and continuous partnership with patients including chronically ill or "high risk" patients and their family/caregiver(s), clinic/hospital/specialty providers and staff and community resources in a team approach to: Promote timely access to appropriate care, Increase utilization of preventative care, Reduce emergency room utilization and hospital readmissions, Promote patient education, Develop and promote adherence to a care plan in coordination with the patient, primary care provider, and family/caregiver(s), Increase continuity of care by managing referrals, Increase patients' ability for self-management and shared decision-making.

Requirements

Graduate of an accredited school of registered nursing.
Must have and maintain a current unrestricted Minnesota Registered Nursing license and CPR certification.
Strong communication skills with the ability to communicate professionally and effectively with clients of all ages, physicians, and coworkers (verbal and written)
