About The Position

McGregor PACE (Program of All-inclusive Care for the Elderly) is a community-based service program that provides in-home healthcare services to older adults as an alternative to nursing home placement, enabling seniors to remain at home. The Clinic Licensed Practical Nurse supports day-to-day clinic operations and provides direct nursing care to participants under the direction of a registered nurse.

Requirements

• Practical Nurse licensed to practice in Ohio.
• Certified in pharmacology (CLPN).
• At least three years of experience with medication administration.
• Current Ohio driver's license and reliable transportation.

Nice To Haves

• Experience in community health, long-term care, home care, or adult day care.
• Previous experience caring for older adults and in dementia care.
• Previous experience in a multidisciplinary care setting.
• CPR certification.

Responsibilities

• Within the LPN scope of practice and pharmacology certification parameters, prepare and administer medications to participants, including oral and parenteral delivery, and perform wound care as required.
• Within Ohio pharmacy law and the LPN scope of practice, prepare medications for participants to take home and use.
• Support the multidisciplinary team by completing initial comprehensive participant assessments by the enrollment date.
• Assist with coordinating and documenting 24-hour care delivery for participants, incorporating input from all disciplines on the multidisciplinary team.
• Conduct initial and ongoing clinical observation of participants under RN oversight, focusing on physical, mental, or emotional changes; report changes to the multidisciplinary team and reflect them in the plan of care as needed.
• Assist with departmental and participant outcome data collection and analysis.
• Participate in performance improvement initiatives as relevant.
