The Clinical Coordinator is a multifunctional position with aspects of care management, risk assessment, and quality assurance analysis and implementation. The Clinical Coordinator will be responsible for collaborating, facilitating, and providing structure to ensure evidence-based quality improvement, manages collection and analysis of clinical outcomes data in order to develop clinical process improvement initiatives, and be able to extract patterns from large sets of data in order to deliver business insights from data. Evaluate patient care data to ensure that care is provided in accordance with clinical guidelines and organizational standards, responds to and investigates any misconduct, noncompliance, or pattern of risk that is detected and collaborates with committee to develop and recommend a plan to cure the non-compliance or risk. Oversee post-discharge activities in order to ensure continuity of care as patient’s transition from inpatient to community. Identify patients that will benefit from case management. Provide ongoing education and support to medical providers and nursing staff to support improvement initiatives. Ensure that training and competency measurements are implemented for staff.
