About The Position

The Clinic Case Coordinator works closely with the clinicians to assist patients with their non-medical needs related to care coordination. The role would interface with patients, parents, referring clinicians, and community partners to concordant communication to and from the clinical team. The role reports to the department manager and works closely with the clinical team. The role works both independently and in conjunction with the clinician team to address complex patient needs in the outpatient clinic and inpatient unit.

Requirements

Bachelor's degree
Two years' experience in a health care setting
Knowledge and understanding of health diagnoses
Critical thinking skills
Psychosocial assessment skills
Knowledge of hospital and community resources
Computer knowledge and ability to effectively utilize and document patient information in the electronic medical record
Knowledge of medical terminology
Demonstrated customer service and organizational skills
Effective listening skills
Ability to show empathy
Applies cultural competence when working with individuals and teams from diverse backgrounds
Knowledge of insurance requirements
Ability to interact with various cultures served by Children's in a culturally competent manner
