Requirements

  • Bachelor's degree in social work or human services field.
  • Employment at Health Connect America and it's companies is contingent upon meeting the requirements of a comprehensive background investigation prior to joining our team.

  • Achieve FCT certification and complete all required trainings within established timeframes to maintain program eligibility.

Responsibilities

  • Serve as clinical lead for the IFI treatment team, providing supervision and guidance to team members.
  • Deliver intensive, in-home and community-based therapeutic services to children, adolescents, and families.
  • Implement the Family Centered Treatment (FCT) model with fidelity, including facilitating interventions across all four phases.
  • Complete all required documentation, fidelity tools, and participate in ongoing supervision and model reviews.
  • Achieve FCT certification and complete all required trainings within established timeframes to maintain program eligibility.
  • Provides community-based services to support children, adolescents, and adults in achieving and maintaining rehabilitative progress.
  • Helps individuals with mental health diagnoses build coping skills, improve daily functioning, and increase self-sufficiency.
  • Connects clients to community resources, develops independent living skills, and supports long-term stability and recovery.
