About The Position

This role is part of the RW Support Services team, focusing on assisting team members in engaging and supporting clients in our programs. The specialist will ensure clients are linked to necessary services and will incorporate best practices regarding confidentiality in all job duties and communications, adhering to HIPAA, Article 27-F, Alliance for Positive Health policies, and other applicable regulations, including the HIV Confidentiality Law. The position involves active participation in the service team, including client case reviews and conferences.

Requirements

  • High School diploma or GED with some lived experience that relates to the target populations.
  • Must have valid, unrestricted driver’s license and a reliable vehicle to travel to designated sites, in the counties we serve and outreach to.
  • Ability to work flexible hours as this position may require evening and /or weekend hours.
  • Strong interpersonal and communication (oral and written) skills.
  • Ability to interact, work and communicate effectively with people of diverse backgrounds.
  • Ability to form community liaisons.
  • Organization skills: ability to work autonomously, good time management skills.
  • Ability to maintain professional boundaries within agency guidelines, and with clients and external service providers.

Responsibilities

  • Work as a member of the RW Support Services team to use their personal experiences to assist team members in engaging and supporting clients in our programs and to ensure they are linked to the services they need
  • Incorporate best practices regarding confidentiality into all job duties and communications in accordance with HIPAA, Article 27-F, Alliance for Positive Health policies and procedures and other applicable regulations. Observe and abide by the HIV Confidentiality Law.
  • Participate as a member of the service team
  • Participate in regular client case reviews and case conferences as applicable.
  • Assist clients in self-advocacy and self-management skill-building, targeted outreach to increase engagement of designated target populations, conduct follow up to service plan activities including appointment reminders and appointment accompaniment.
  • Communicate and coordinate with team members regarding assigned clients.
  • Perform other duties as assigned.
