About The Position

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency operating throughout Arizona, offering residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. CBI is recognized as a premiere non-profit provider of substance use and behavioral health programs, utilizing cutting-edge, nationally recognized, evidence-based models. The Client Stabilization Specialist position is based at a shelter or temporary lodging site, primarily working with individuals and families experiencing homelessness. The role focuses on stabilizing new entrants, conducting intakes into HMIS, educating on rules, responsibilities, and resources, and providing overall orientation. Responsibilities also include face-to-face visits, perimeter walks, and escalating issues that impact stabilization. The specialist is responsible for communicating with leadership and Client Care Coordinators regarding program violations to ensure issues are addressed, and for reporting identified complex needs (substance use, mental health, housing, medical care) to leadership for proper service coordination. Additionally, the position involves data entry into established systems for violations, end-of-shift notes, and room checks, and identifying the need for housing prioritization assessments for all individuals.

Requirements

  • Highschool diploma or GED is required.
  • Minimum of twelve months of recovery from substance use and/or mental health disorders required.
  • Valid identification required.
  • Will complete CBI Peer Certification within 90-days of being hired.
  • Arizona Fingerprint Clearance card (program specific).

Nice To Haves

  • 1-3 years of full-time health care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations is preferred.
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures is preferred, but not required.

Responsibilities

  • Working with individuals and families who are experiencing homelessness.
  • Ensuring individuals or families who are entering shelter or temporary lodging are stabilized.
  • Conducting intakes into HMIS.
  • Providing education on the rules and responsibilities, resources, and overall orientation while in shelter or temporary lodging.
  • Conducting face-to-face visits.
  • Conducting perimeter walks.
  • Escalating issues impacting the stabilization of individuals or families in shelter or temporary lodging.
  • Ensuring communication with leadership and Client Care Coordinators on program violations to ensure issues are addressed.
  • Communicating with leadership any identified needs of individuals who present with complex substance use, mental health, housing, and medical care needs to ensure the proper coordination of services.
  • Completing data entry into established data system, including, violations, end of shift notes, and room checks.
  • Identifying the need for completion of a housing prioritization assessment for all individuals.
