About The Position

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency operating throughout Arizona, offering residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. CBI is a non-profit provider utilizing cutting-edge, evidence-based models. The Client Stabilization Specialist position is based at a shelter or temporary lodging site, primarily working with individuals and families experiencing homelessness. The role ensures stabilization for those entering shelter or temporary lodging by conducting intakes into HMIS, providing education on rules and responsibilities, offering resources, and giving overall orientation. The specialist is responsible for face-to-face visits, perimeter walks, and escalating issues impacting stabilization. Communication with leadership and Client Care Coordinators is essential for addressing program violations and facilitating stabilization. The specialist also communicates identified needs of individuals with complex substance use, mental health, housing, and medical care needs to ensure proper service coordination. Additionally, the position involves completing data entry into established data systems, including violations, end-of-shift notes, and room checks, and identifying the need for housing prioritization assessments for all individuals.

Requirements

  • Highschool diploma or GED is required.
  • Minimum of twelve months of recovery from substance use and/or mental health disorders required.
  • Valid Identification required
  • Will complete CBI Peer Certification within 90-days of being hired.
  • Arizona Fingerprint Clearance Card (program specific)

Nice To Haves

  • 1-3 years of full-time health care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations is preferred.
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures is preferred but not required.

Responsibilities

  • Work with individuals and families experiencing homelessness to ensure stabilization upon entering shelter or temporary lodging.
  • Conduct intakes into HMIS.
  • Provide education on the rules and responsibilities, resources, and overall orientation while in shelter or temporary lodging.
  • Conduct face-to-face visits and perimeter walks.
  • Escalate issues impacting the stabilization of individuals or families in shelter or temporary lodging.
  • Ensure communication with leadership and Client Care Coordinators on program violations to ensure issues are addressed.
  • Communicate with leadership any identified needs of individuals who present with complex substance use, mental health, housing, and medical care needs to ensure the proper coordination of services.
  • Complete data entry into established data system, including violations, end of shift notes, and room checks.
  • Identify the need for completion of a housing prioritization assessment for all individuals.
