Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona, including residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. CBI is a premiere non-profit provider of substance use and behavioral health programs, utilizing cutting-edge, nationally recognized, evidence-based models. The Client Stabilization Specialist position is a shelter or temporary lodging site-based role primarily focused on working with individuals and families experiencing homelessness. The specialist will ensure stabilization for individuals or families entering shelter or temporary lodging, conduct intakes into HMIS, and provide education on rules, responsibilities, resources, and overall orientation. Responsibilities include face-to-face visits, perimeter walks, and escalating issues affecting stabilization. The specialist will communicate with leadership and Client Care Coordinators regarding program violations and with leadership about complex needs (substance use, mental health, housing, medical care) to ensure proper service coordination. The role also involves data entry into established systems for violations, end of shift notes, and room checks, and identifying the need for housing prioritization assessments.
