About The Position

The Client Relations Associate (CRA) is a field-based business development representative responsible for meeting referral and admission expectations. This role focuses on maintaining and growing market share and identifying new business opportunities to support the branch's business plan. The CRA works with the Agency Director and Director of Business Operations to develop and execute strategies for revenue targets, patient experience, and profitability. Primary customers include skilled nursing facilities, assisted living communities, community-based residential facilities, physicians, professional associations, and organizations within the established market.

Requirements

  • Health care professional credential, Bachelor’s degree or comparable business experience
  • Minimum of one year of related sales experience, preferably in a health care/medical field or patient transportation

Responsibilities

  • Supports CHI Health at Home’s mission to exceed customer, associate, and shareholder expectations in healthcare delivery.
  • Supports CHI and CHI Health at Home’s values, strategic goals, and high standards of customer service.
  • Lives People First Behaviors and follows organizational policies and procedures.
  • Adheres to the Corporate Compliance Program, including HIPAA confidentiality.
  • Meets or exceeds established monthly admission targets.
  • Identifies and qualifies healthcare relationships within a regional territory, focusing on skilled nursing facilities, assisted living communities, community-based residential facilities, physicians, and other healthcare providers and hospital systems.
  • Develops and maintains relationships with key customer sources through regular contact and follow-up.
  • Acts as the key contact for client troubleshooting and conflict resolution.
  • Develops and works a yearly business plan for the region, including strategies for exceeding budgets and goals.
  • Maintains accurate data on customer relationships within RSL for each key referral source.
  • Communicates daily with agencies to exchange vital client information.
  • Completes weekly reports detailing field activity and results.
  • Represents CHI Health at Home in relevant professional organizations and the community.
  • Assists in proposal preparation and presentation.
  • Analyzes and makes recommendations for contracts.
  • Assists in educating the community about services.
  • Follows the referral of a patient to CHI Health at Home.
  • Assists facility discharge coordinators in coordinating quality home care services.
  • Determines patient eligibility for home health or hospice services.
  • Communicates effectively with disciplines involved in patient care and provides information/recommendations to CHI Health at Home staff.
  • Develops professional working relationships with healthcare providers and facilities.
  • Is available to analyze eligibility for home care or hospice and provide guidance on benefits, financing, and policies.
  • Performs other duties as assigned.
