About The Position

The Client Care Coordinator will work collaboratively with the Client Care Manager and other case management staff to provide individualized case management services to eligible clients. This role involves meeting with clients to complete intakes, assessments, income verification, program qualifications, enrollments, ongoing case management, and case closings. The coordinator will also enroll and maintain virtual accounts for case management application services, provide in-person, virtual, and telephonic support to clients, and work with upper management to determine appropriate client relief allocation awards. Additionally, the role includes developing, maintaining, and assessing program materials, providing outreach to the local community, creating and distributing promotional materials, and offering front desk and administrative support as needed. The position requires maintaining strict confidentiality, a strong knowledge of TEEG services and community resources, and accurate data entry and reporting.

Requirements

  • Desire to work with disenfranchised populations while supporting TEEG’s mission
  • Bachelor’s degree in human services or related field or 5 years of similar work experience
  • Proficiency in Microsoft Office software
  • Strong written, oral, organizational, and conflict-resolution skills
  • Ability to work independently and collaboratively with flexible hours and duties
  • General knowledge of local and regional services and supports
  • Clean record with State Police Bureau of Identification, Department of Children and Families and Department of Motor Vehicles
  • Possesses a valid driver’s license, reliable transportation, and proof of registration and motor vehicle insurance

  • Mandated Reporting Certification
  • Elder Abuse Identification & Mandated Reporting Certification
  • CPR & First Aid Certification
  • TEFAP Civil Rights Certification
  • Bloodborne Pathogens Certification

Nice To Haves

  • Prior certification for Mandated Reporting, Elder Abuse Identification & Mandated Reporting, CPR & First Aid, TEFAP Civil Rights, and Bloodborne Pathogens is desirable, but candidates lacking one or more certifications can obtain the certification on the job.

Responsibilities

  • Work collaboratively with Client Care Manager and other case management staff to provide individualized case management services to eligible clients as needed
  • Meet with current and referred clients to complete intakes, assessments, income verification, program qualifications, enrollments, ongoing case management, and case closings as needed
  • Enroll and maintain virtual accounts for available case management application services
  • Provide in-person, virtual, and/or telephonic supports to clients as needed
  • Become CHOICES certified within 6 months
  • Work with upper management to determine appropriate client relief allocation awards
  • Develop, maintain, and assess applicable program materials as needed
  • Provide outreach to the local community.
  • Create and distribute outreach and promotional materials for clients, community resources and public consumption to promote senior programming
  • Provide front desk support as needed
  • Perform administrative support or coverage for other program staff when needed
  • Any other duties as assigned by Supervisor or Executive Director
