Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona, including residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. The Client Care Coordinator is a seasonal, shelter or temporary lodging site-based position, active during peak heat periods from May to October, with potential for continued employment. The primary role is managing a housing caseload, coordinating care for individuals and families experiencing homelessness, and providing support services to help them end their current episode of homelessness. This role involves assisting individuals with complex substance use, mental health, housing, and medical care needs, ensuring coordination with existing or new PCMH/PCP services. The Client Care Coordinator serves as a bridge, coordinating and monitoring RBHA and Non-RBHA community-based services, facilitating their delivery, and working with individuals and families with the highest acuities and greatest vulnerabilities. The role also involves coordinating care through the CBI Continuum of Care (or other community agency) on social determinants of health, including crisis, urgent care, residential treatment, medical care, medication assisted treatment, or outpatient behavioral health services. Once matched to housing, the coordinator uses housing search software to identify units. This position requires data entry into established systems, assisting higher-level personnel with reports, and completing end-of-shift reports. The Client Care Coordinator works as part of a multi-disciplinary team and coordinates care with community providers and stakeholders, documenting interactions according to various regulatory and policy guidelines. In this role, the coordinator delivers lifesaving support to vulnerable populations affected by extreme heat, collaborating with community members, local service providers, and internal teams to distribute resources, share information, and connect people with appropriate services. It requires a strong commitment to community engagement, empathy, and responsiveness to urgent needs.
