Responsibilities

Acting as a trusted point of contact for families seeking home care services
Building care plans by coordinating schedules between caregivers and clients
Supporting smooth onboarding and orientation for new caregivers joining the team
Handling daily updates, service changes, and communication between all parties involved
Visiting or checking in on client situations to ensure care quality and satisfaction
Maintaining accurate records and keeping documentation up to date for compliance
Working closely with office staff to improve service flow and resolve concerns quickly
Participating in outreach activities and helping connect with potential caregivers in the community
Managing scheduling adjustments and solving day-to-day coordination challenges
Attending team discussions and contributing to overall service improvement
