The Children's Long Term Support Service Coordinator assists families in the process of applying for Children’s Long Term Support Services. This role performs intake assessments and interviews collateral sources to determine eligibility for services, providing families with information, support, and opportunities to make informed decisions. The coordinator authorizes and coordinates services to help families care for their children at home, understanding the Service Delivery System, valuing natural supports, and managing budgets and funding effectively. They develop and monitor Individual Service Plans, maximize family self-sufficiency, and document all activities. This position requires building community partnerships, participating in meetings and training, and maintaining flexibility, confidentiality, and punctuality, which may include working unstructured and evening hours.
