About The Position

This position assists families in the process of applying for Children’s Long Term Support Services. It involves performing intake assessments and interviewing collateral sources to determine service eligibility. The role provides families with information, support, and opportunities to make informed decisions. The Service Coordinator authorizes and coordinates individualized comprehensive services to help families care for their children at home. This includes understanding the Service Delivery System, valuing natural supports, and acknowledging budget limitations to ensure resources are effective, efficient, and valuable to participants. The position also involves coordinating and facilitating the development of Individual Service Plans, monitoring progress, and managing allocated funding. It aims to maximize family self-sufficiency by connecting them with relevant community resources. The role requires efficient and effective support and service coordination that reflects the philosophy of self-determination of the Children’s Long-Term Support Program and Children’s Community Options Program. Documentation of all activities and completion of required paperwork are essential. Building and maintaining positive community partnerships, participating in meetings, and engaging in training opportunities to enhance skills are also key aspects. Availability and responsiveness to families and team members, maintaining flexibility, confidentiality, and punctuality, including working unstructured and evening hours, are expected.

Requirements

  • Bachelor’s degree in Social Work or related Human Services field
  • One year of experience working with families who have children with disabilities.
  • Ability to adjust/cope with a constant caseload demand, efficiently prioritizing caseload and service needs to ensure priority services are delivered.
  • Demonstrated history to skillfully plan and organize work to achieve the most effective delivery system.
  • Ability to communicate effectively and professionally, both verbally and in writing.
  • Ability to use a county issued computer for Microsoft programs, internet, and program specific database management systems.
  • General math knowledge to monitor service logs, expense forms, invoices, and authorizations.
  • Ability to facilitate and participate in meetings with a variety of people in an array of settings.
  • Demonstrated ability to be a self-starter.
  • Flexible in meeting the varied needs of families and workload.
  • Must possess a valid driver’s license and have access to a reliable, insured vehicle in order to provide services in a variety of settings.
  • Ability to work unstructured hours, including evening hours.
  • Ability to perform the physical requirements of the job necessary for this target group.

Responsibilities

  • Assists families in the process of applying for Children’s Long Term Support Services.
  • Performs intake assessments and interviews collateral sources to determine if individual is eligible for services.
  • Provides families with information, support, and opportunities as needed to enable them to make informed decisions.
  • Authorizes and coordinates services to assist families to care for their children in their home.
  • Provides and coordinates individualized comprehensive services to assist families in caring for their children in their homes.
  • Understands the Service Delivery System, values natural supports, acknowledges the limitations of budgets, funding, costs, and choices; ensuring resources are effective, efficient, and have value to the participant.
  • Coordinates and facilitates the development of Individual Service Plans that meet the needs of families and maintain measures of progress in meeting outcomes.
  • Conducts on-going monitoring and targeted case management practice.
  • Creates a budget and manages funding allocated to each case.
  • Reviews invoices and service codes and authorizes invoices for payment.
  • Maximizes and supports the family’s ability to be self-sufficient while accessing community resources relevant to their needs.
  • Provides Support and Service Coordination Services efficiently and effectively which reflects the philosophy of self-determination of the Children’s Long-Term Support Program and Children’s Community Options Program.
  • Documents all support and service coordination activities and completes paperwork as required.
  • Works to build and maintain positive community partnerships.
  • Participates in meetings and training opportunities to enhance skills.
  • Is available and responsive to families and team members by maintaining flexibility, confidentiality and punctuality.
  • Ensures each participant's legal rights, safeguards, and confidentiality are upheld.
  • Ensures compliance with Federal, State, County, and department rules, regulations, policies, and procedures.
  • Provides accurate and complete assessment/applications, progress notes, and other written information as required in accordance with State, Federal, and department standards and schedules.
  • Meets on-going training requirements of funding sources and maintains knowledge necessary to assess, plan, and monitor supports and to keep current in rules, regulations, and community resources.
  • Responds to families, service providers, and co-workers in a timely and courteous manner.
  • Ensures the implementation of department values, goals, and procedures.
  • Demonstrates professional conduct.
  • Mobilizes appropriate resources that are effective, efficient, and have value to those that use funding or services.
