Requirements
- Leadership experience in a 1000+ census home health agency.
- Success leading multi-site clinical operations.
- 5-star rating history for both outcomes and HHCAHPS.
Responsibilities
- Create one clinical vision across all care divisions.
- Standardize processes, treatment plans, quality metrics and training.
- Build AI-enabled systems that make care management smarter.
- Develop the roadmap for IE-SNP readiness.
