About The Position

Belleview Care Center is seeking a compassionate, dedicated Charge Nurse (RN/LPN) to provide excellent care to residents in a warm, supportive environment. The nurse provides resident care, complies with state and federal regulations, collaborates with the healthcare team, coordinates care across disciplines, and assesses and coordinates discharge-planning needs with healthcare-team members. Full-time and PRN opportunities are available, with 12-hour shifts on a rotating schedule from 7 a.m. to 7 p.m. or 7 p.m. to 7 a.m.

Requirements

• Valid state LPN or RN license(s).
• Ability to work independently and efficiently as a self-starter.
• Reliable transportation.

Responsibilities

• Provide patient care to residents.
• Comply with state and federal regulations.
• Work collaboratively with members of the healthcare team.
• Serve as a primary coordinator of all disciplines to support well-coordinated patient care.
• Assess and coordinate patients’ discharge-planning needs with members of the healthcare team.
