About The Position

North Adams Commons Nursing & Rehab Center, part of Integritus Healthcare, provides rehabilitation, short-term care, skilled nursing, long-term care, respite, and hospice services. The Charge Nurse (RN or LPN) administers and coordinates resident care, completes clinical and administrative documentation, and leads nursing personnel and nursing assistants to support residents’ care needs and maintain nursing standards.

Requirements

• Graduate of an accredited nursing program.
• Current Massachusetts nursing license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
• CPR/BLS/AED and IV certifications required.

Responsibilities

• Administer medications and treatments according to physician orders.
• Manage resident admissions, transfers, and discharges.
• Coordinate discharge planning for nursing services as needed.
• Collaborate with resident services to maintain residents’ total regimen of care.
• Foster a positive atmosphere of warmth and personal interest.
• Complete administrative tasks, including medical forms, reports, evaluations, and charting.
• Transcribe physician orders to resident charts, cardex, medication cards, and care plans.
• Maintain routine charting in accordance with documentation policies.
• Document accidents and incidents by completing required reports and charts.
• Manage financial paperwork, including Daily Census Reports and resident charge slips for the Business Office.
• Update dietary changes and forward new orders to the Dietary Department.
• Lead assigned nursing personnel and direct nursing assistants under long-term care regulations.
• Conduct regular rounds to ensure staff meet nursing performance standards.
• Host shift meetings with nursing and support staff to plan services and activities.
• Enforce compliance with written guidelines and the Nursing Service Procedures Manual.
• Verify that personnel comply with their specific job descriptions.
• Monitor inventory to ensure adequate medications, supplies, and equipment are available.
