About The Position

The Community Family Resource Center (CFRC) Case Manager will oversee a variety of projects and activities aimed at implementing a comprehensive client engagement and service delivery system. The Case Manager will be responsible for providing case management services to CFRC clients. Additionally, the Case Manager will coordinate services that may be delivered in the home, within the community, or virtually, depending on the needs of the family. Key duties include communicating with both internal and external stakeholders, offering timely referrals and linkages to community-based services for individuals and families, and responding to walk-in inquiries for information. The Case Manager will assist caregivers and families in navigating complex systems, empowering them to advocate for themselves and the wellbeing of their children. The role also includes facilitating and participating in various groups and classes as needed. The Case Manager must reside in the supervisor district (SD) that they are serving or be willing to relocate. The Case Manager follows all Penny Lane Centers policies and procedures regarding the Heart Centered Culture, cultural sensitivity, and cultural awareness. The Case Manager supports the agency mission and vision in their interaction with children, youth, families, and partner agencies.

Requirements

  • Must reside in the supervisory district (SD) they are serving or be willing to relocate.
  • Valid California driver’s license.
  • Current automobile insurance.
  • Have and maintain a clean driving record acceptable to the organization’s insurance company.
  • Have immediate access to his or her vehicle during work hours.

Nice To Haves

  • Bachelor’s Degree preferred, with experience in the provision of direct services to low income and ethnically diverse clients.
  • If no bachelor’s degree, a High School diploma or GED preferred and a minimum of two years’ experience in the provision of direct services to low income and ethnically diverse clients.
  • Bilingual Spanish preferred (verbal and written).
