About The Position

The Fortune Society, in partnership with Community Healthcare Network (CHN), provides court- and community-based, person-centered, voluntary support to people with unmet needs returning to the community after arraignment at Brooklyn Criminal Court or other Brooklyn court parts. The program connects participants with peer support and clinical services through CHN’s mobile medical unit (MMU). The Certified Peer Navigator supports participants in identifying and addressing medical, behavioral health, and social needs; connects them with appropriate services; and follows up on engagement. The role includes working with court stakeholders, Fortune court advocates, peers, and MMU clinicians, as well as supervising and mentoring peer team members. This is an on-site growth position. The posting also describes it as an opportunity for new MSW graduates interested in potential movement into Fortune outpatient treatment programs after receiving licensure.
