About The Position

VitalCaring is seeking a Certified Occupational Therapy Assistant (COTA) to provide field-based, one-on-one home health care. The role helps patients regain independence and confidence at home by rebuilding skills for daily living after illness or injury. The COTA works under the direction of a Licensed Occupational Therapist and delivers personalized, goal-driven care while managing their day with support from an experienced OT team. The position collaborates with nurses, therapists, and leadership to support patient functional outcomes and quality of life.

Requirements

• Graduate of an accredited Occupational Therapy Assistant program.
• Active COTA license or certification in the state of practice.
• Current CPR certification.
• Ability to work independently in a field-based environment.
• Strong clinical observation, communication, and time management skills.
• Valid driver’s license, reliable transportation, and auto insurance.

Nice To Haves

• Home Health experience is strongly preferred.
• Experience delivering therapy in patient homes rather than facility settings.
• Ability to adapt care plans to real-life home environments.
• Confidence identifying and escalating changes in patient condition.
• A balance of clinical skill and compassionate care.

Responsibilities

• Implement treatment plans to improve patients’ ability to perform daily activities.
• Provide hands-on therapy focused on strength, coordination, mobility, and self-care.
• Retrain patients in activities of daily living, including dressing, bathing, and meal preparation.
• Assess and report on patient progress, function, and home safety needs.
• Educate patients and caregivers on techniques, exercises, and safety strategies.
• Identify changes in patient condition and communicate proactively with the OT and care team.
• Complete timely, accurate documentation in the EMR.
• Support a safe, effective, and coordinated plan of care.
