About The Position

Flagstar Rehab is staffing a Certified Occupational Therapy Assistant for its Rehabilitation Department in a skilled nursing facility in Briarcliff Manor, New York. The role involves providing therapy services and patient and family education to help improve patients’ physical abilities, cognition, and daily functioning. The position offers direct employment with Flagstar Rehab or the option to work as an independent contractor; the agency can assist with setting up a corporation at no charge.

Pay is $34–$36 per hour for W-2 employees or $38–$40 per hour for 1099 contractors. Schedules may be full-time, part-time, per diem, weekends, coverage, days, or evenings.

Requirements

• Current New York State OTA license and registration.
• Competence with electronic medical records (EMR).
• State-required medical documentation for work in a skilled nursing facility, including a physical, TB test, MMRV vaccine record or titers, and COVID-19 vaccine documentation.
• Valid proof of identity and work authorization.
• Malpractice/liability insurance, if applicable.

Responsibilities

• Collaborate daily with the OT Supervisor and follow each patient’s established Plan of Care.
• Provide quality therapy services to skilled nursing facility patients with conditions including TBI, CVA, stroke, Parkinson’s disease, multiple sclerosis, cancer, orthopedic surgery, and other debilitating diagnoses.
• Monitor and document patient progress using EMR software.
• Provide patient and family education to support patients’ physical abilities, cognition, and daily functioning.
