About The Position

As a Certified Occupational Therapy Assistant (COTA) in our long-term care facility located near Bloomington, IN you will work closely with licensed occupational therapists to provide comprehensive rehabilitation services to our residents. Your role will involve assisting in the implementation of individualized treatment plans, conducting therapeutic interventions, and promoting residents' independence in activities of daily living (ADLs) and functional skills.

Requirements

  • Associate degree from an accredited Occupational Therapy Assistant program.
  • Current certification as a Certified Occupational Therapy Assistant (COTA)
  • Valid state licensure or eligibility for licensure as a COTA in Indiana
  • Strong interpersonal skills, compassion, and a commitment to person-centered care.
  • Ability to work collaboratively in a team environment and communicate effectively with residents, families, and colleagues.

Nice To Haves

  • Previous experience in long-term care or geriatric rehabilitation preferred.

Responsibilities

  • Assist in the assessment of residents' functional abilities, limitations, and needs under the supervision of licensed occupational therapists.
  • Implement therapeutic interventions and activities as outlined in residents' treatment plans to address deficits and promote optimal functional outcomes.
  • Provide hands-on assistance and guidance to residents during ADLs, mobility training, and other rehabilitation activities to enhance independence and safety.
  • Monitor residents' progress, document treatment sessions, and communicate effectively with the therapy team to ensure coordinated care and continuity of services.
  • Educate residents, families, and caregivers on adaptive techniques, equipment use, and home modifications to support continued recovery and independence.
  • Collaborate with interdisciplinary team members, including therapists, nurses, social workers, and other healthcare professionals, to optimize resident care and outcomes.
  • Participate in Home Health Patient meetings, in-service trainings, and quality improvement initiatives to uphold clinical excellence and regulatory compliance.
  • Support discharge planning efforts by assessing residents' readiness for transition and assisting in the coordination of post-discharge services and follow-up care.
