Certified Occupational Therapist Assistant (COTA)
About The Position
Ultimate Therapy is seeking a full-time Certified Occupational Therapy Assistant (COTA) at Autumn Lake at Scotch Plains in Scotch Plains, New Jersey. The assistant works alongside an Occupational Therapist to help skilled nursing residents regain daily living skills, learn adaptive approaches, and participate more confidently in their care. Treatment is guided by each resident’s established occupational therapy plan, with progress and changes communicated to the supervising OT. The role also involves accurate treatment documentation and collaboration with nursing and therapy colleagues to support consistent care strategies.
Requirements
- Active New Jersey Occupational Therapy Assistant license and COTA certification.
- Ability to follow individualized care plans and resident-specific precautions.
- Effective communication, organization, and documentation skills.

- Skilled nursing experience is a plus; new graduates are welcome to apply.
Responsibilities
- Provide hands-on treatment guided by each resident's established occupational therapy plan.
- Practice personal care routines with residents, tailoring instruction and assistance to their needs.
- Lead prescribed exercises and functional tasks addressing upper-extremity abilities, coordination, and activity tolerance.
- Reinforce techniques for adaptive equipment, safe task completion, and energy conservation.
- Communicate residents' progress and changes in performance or participation to the supervising Occupational Therapist.
- Complete accurate treatment documentation.
- Collaborate with nursing and therapy colleagues on consistent care strategies.
