About The Position

Concept Rehab is seeking a Certified Occupational Therapy Assistant (COTA / OTA) to join our team at a skilled nursing facility (SNF) in Avon, Ohio. We offer flexible Full-Time, Part-Time, and PRN schedules, making this a great opportunity for clinicians seeking stability, flexibility, or supplemental income. Requirements

• Graduate of an accredited Occupational Therapy Assistant program
• Active Ohio COTA license (or eligible)
• Strong communication and teamwork skills
• Passion for working with geriatric and rehab populations

Nice To Haves

• Skilled Nursing Facility (SNF), rehabilitation, or post-acute care experience
• Experience with EMR/EHR documentation systems
• Experience in Skilled Nursing Facilities (SNF), Long-Term Care (LTC), Post-Acute Rehabilitation, Assisted Living, Home Health, or Outpatient Therapy settings

Responsibilities

• Deliver occupational therapy treatments under the supervision of an OTR
• Implement individualized care plans focused on ADLs and functional independence
• Collaborate with therapists, nursing staff, and families
• Maintain accurate and timely electronic documentation
• Educate patients, caregivers, and staff on therapy programs
• Assist with discharge planning and patient progress tracking
