About The Position

Trellis Location: Chino, CA Trellis is a beautiful skilled nursing facility located in Chino, California. We are seeking a dedicated and compassionate Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team. This position supports quality patient care by implementing individualized treatment plans under the supervision of a licensed Occupational Therapist. Position Purpose: To assist in delivering high-quality, patient-centered occupational therapy services in accordance with established treatment plans, facility policies, and regulatory standards.

Requirements

  • Associate degree or higher in Occupational Therapy from an AOTA or ACOTE-accredited program
  • Current certification and good standing with all applicable regulatory agencies and licensing boards
  • Knowledge of resident rights and commitment to patient confidentiality
  • Ability to read, write, speak, and understand English effectively
  • Strong interpersonal skills and ability to maintain professional working relationships
  • Demonstrated integrity and professionalism; adherence to company Code of Conduct and compliance policies
  • Completion of all mandatory compliance and ethics training

Responsibilities

  • Implement and carry out patient treatment plans under the supervision of a licensed Occupational Therapist
  • Communicate effectively and professionally with patients, families, physicians, and interdisciplinary team members
  • Provide timely, accurate written documentation in accordance with facility and departmental requirements
  • Report any changes in patient status or treatment needs to the supervising therapist promptly
  • Coordinate treatment goals with other members of the care team
  • Accurately prepare and submit patient charges to support appropriate billing
  • Support departmental operations, initiatives, and quality improvement efforts
  • Continue developing clinical and professional skills through ongoing education and training
