About The Position
Trellis Chino is a 59-bed short-term skilled nursing facility seeking compassionate and dedicated Licensed Vocational Nurses (LVNs) to join their NOC shift team. They offer a patient-centered approach focused on providing exceptional clinical care and outstanding resident experiences. This opportunity is open to both experienced nurses and recent graduates.
Requirements
- Current California LVN license in good standing
- Strong clinical assessment and documentation skills
- Excellent communication and interpersonal abilities
- Ability to work independently and collaboratively within a team environment
- Compassionate and resident-focused approach to care

Responsibilities
- Administer medications and treatments according to physician orders
- Accurately document patient care using electronic medical records
- Monitor and assess resident conditions, reporting changes as appropriate
- Provide care for residents with acute and chronic healthcare needs
- Supervise and support Certified Nursing Assistants (CNAs)
- Communicate effectively with residents, families, physicians, and interdisciplinary team members
- Utilize critical thinking skills to make sound clinical decisions
- Ensure compliance with all facility policies, procedures, and regulatory requirements
