About The Position

HealthPro Heritage is seeking a Certified Occupational Therapy Assistant (COTA) to join the rehabilitation team at Sage Post Acute in Hayward, California. The position supports occupational therapy services in a skilled nursing setting, helping residents regain function, independence, and confidence in daily life. New graduates are encouraged to apply. Requirements

Associate’s degree in an Occupational Therapy Assistant program from an accredited school.
Certification as an Occupational Therapy Assistant (COTA) through the National Board for Certification in Occupational Therapy (NBCOT).
Current state licensure, or eligibility for licensure, to practice as a Certified Occupational Therapy Assistant.

Previous experience in an occupational therapy setting, such as a hospital, clinic, or school; new graduates are also encouraged to apply.

Responsibilities

Assist in delivering individualized therapy based on treatment plans developed by the Occupational Therapist, including activities to improve daily living skills such as dressing, eating, and mobility.
Track and document patient progress, maintain accurate records in compliance with facility policies, and report changes or concerns to the Occupational Therapist.
Collaborate with interdisciplinary teams, including physical therapists, occupational therapists, and nursing staff, to provide holistic care.
Educate patients and families on therapeutic exercises, adaptive equipment, and strategies for independence.
Lead or assist patients with exercises, activities of daily living, and other therapeutic tasks while encouraging engagement in a positive, supportive environment.
