About The Position

The Certified Nursing Assistant provides hands-on care to patients in a skilled nursing setting. The role assists patients with activities of daily living and supports their safety, comfort, dignity, and independence. The CNA works with the care team to follow individualized care plans, document care, communicate changes in patient condition, and maintain a clean and safe environment.

Requirements

• Active Certified Nursing Assistant (CNA) certification.
• Ability to lift, move, and transfer patients safely.
• Strong teamwork, reliability, and a patient-first mindset.
• Ability to read, write, and speak English.

Responsibilities

• Provide direct patient care, including bathing, dressing, grooming, toileting, and feeding.
• Assist with patient transfers, ambulation, and repositioning using safe lifting techniques.
• Monitor and observe patients and report changes in condition to the Charge Nurse.
• Document patient care and daily activities accurately and in accordance with regulations.
• Support and follow individualized care plans to improve patient outcomes.
• Maintain a clean, safe, and infection-controlled environment.
• Ensure medical equipment and devices are clean, charged, and functioning properly.
• Participate in shift handoffs, team communication, and ongoing training.
• Follow state, federal, and facility policies and procedures.
• Promote patient dignity, independence, and safety at all times.
