About The Position

Moments Hospice is seeking a Certified Nursing Assistant (CNA) to support patient comfort and dignity. The CNA performs personal care and activities of daily living, assists with mobility and nutrition, reviews care plans, and maintains electronic medical records under the direction of a Case Manager RN. The role offers collaboration with a dedicated RN Case Manager, structured training, reasonable caseloads, and on-call support. The position is on the road and includes a company car option for business and personal use, with fuel and insurance covered at minimal cost to the employee.

Requirements

• Certified Nursing Assistant (CNA) license.
• At least 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital, or Hospice.
• Excellent customer service skills.
• Strong written and verbal communication and active listening skills.
• Problem-solving skills.
• Time management skills.
• Ability to work in an electronic medical record system.
• Ability to lift 50 lbs.
• Good understanding of body mechanics.
• Strong understanding of professional boundaries.
• Valid driver's license.
• Valid auto insurance.

Nice To Haves

• Current CPR certification.

Responsibilities

• Perform personal care activities related to daily living, including bathing, dressing, grooming, toileting, and skin care.
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
