About The Position

Moments Hospice is seeking a Certified Nursing Assistant (CNA) to provide personal care that supports patient comfort and dignity. The CNA performs activities of daily living, assists with mobility, positioning, exercise, dining, and nutrition, and maintains electronic medical records. The role works under the direction of a Case Manager RN and includes collaboration with an RN Case Manager, structured training, clear expectations, reasonable caseloads, and on-call support. The position is described as on the road. Compensation is $18–$23 per hour. A company vehicle option is available for business and personal use at minimal cost, with fuel and insurance covered.

Requirements

• Current Certified Nursing Assistant (CNA) license.
• At least 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital, or Hospice.
• Excellent customer service skills.
• Strong written and verbal communication and active listening skills.
• Problem-solving skills.
• Time management skills.
• Ability to work in an electronic medical record system.
• Ability to lift 50 lbs.
• Good understanding of body mechanics.
• Strong understanding of professional boundaries.
• Valid driver's license.
• Valid auto insurance.

Nice To Haves

• Current CPR certification.

Responsibilities

• Perform personal care activities related to daily living, including bathing, dressing, grooming, toileting, and skin care.
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
