About The Position
Encore Northwoods Lodge is a 57-bed facility specializing in short-term skilled nursing care. The Certified Nursing Assistant provides personal care to patients in their residence under the direction, instruction, and supervision of a registered nurse. The role supports patients during admissions, transfers, and discharges, and helps ensure their personal care needs are met in accordance with their wishes and care plans. Requirements
- CNA license certification is required prior to hiring.
- Current CPR certification is required.
- Current fingerprint clearance card is required.
Nice To Haves
- At least six months of CNA/LNA experience is preferred.
- Recent long-term care, skilled nursing facility, or transitional rehabilitation experience is preferred.
- LTC/SNF/rehabilitation experience is preferred.
Responsibilities
- Provide assigned personal care to patients under the direction, instruction, and supervision of the registered nurse.
- Assist patients during admissions, transfers, and discharges.
- Routinely check that patients’ personal care needs are being met in accordance with their wishes and care plans.
- Review care plans daily to identify changes to patients’ daily care routines.
